- Liaison between MHSI patients and the health center staff, as well as between the health center staff and the community
- Works with the health center staff to provide services for farmworkers, including:
- Need assessments for referral to other agencies or for planning health education services
- Referrals to other community resources
- Health education information
- Arranging support groups on topics of patients' interest
- Helps patients apply for Minnesota Health Care programs (Medical Assistance, MinnesotaCare, and others), as well as drug companies' prescription assistance programs
- Available to help patients apply for county cash assistance and food support
